This plan isn’t insurance, so there are no premiums, claims, deductibles, or other headaches that are involved with dental insurance. Consider it the dental insurance alternative. Basically, the annual fee covers most preventive services in full and gives you a discount on many other restorative and periodontal procedures. Discounts will be given for services not listed/not included. 20% discount, if paid with check or cash, 17% discount, if paid with credit card, or 5% discount if paid with care credit, on services not listed/not included. EXCLUDES ORTHO/INVISALIGN. Services must be paid for in full on date of service to receive discount. No exceptions.
New and returning patients will be charged a discounted initial exam fee of $48 in addition to the Membership Plan cost. The exam fees are included in the Membership Plan for existing patients. Any patient who does not have dental insurance can purchase a membership. Memberships are not considered insurance. All dental care must be provided by Market Place Dental Group. Membership benefits are not transferable to another dental office. It’s the member’s responsibility to schedule and keep all appointments that are offered in this program. Memberships are good for one year from the date of purchase. All benefits must be used within the coverage period. Any unused services cannot be carried over to the New Coverage Year. Memberships begin on the date of purchase. Membership fees are to be paid in full at the time of enrollment and are non-refundable, no exceptions. Payments can be in the form of cash, check, credit card, or care credit. No additional discounts apply to membership fee.
